Help with a Federal Agency

***Special Instructions Regarding Passports***

The Passport Agency is facing an extremely high level of demand nationwide and is making every effort to assist travelers based on their planned departure date.

Please note that my office cannot guarantee the issuance of a passport or the scheduling of an appointment by a specific date. We will work with the agency to accommodate you as best as possible.

Per Department of State policy, to be eligible for passport assistance from my office, you must have a booked departure date in the next 14 days. The Passport Agency will not accept inquiries about travel beyond this period.

To be of assistance, I will need the following:

  • A signed Department of State privacy release form for each applicant
    • Be sure to include the last four digits of the applicant's social security number (SSN) and date of birth (DOB)
    • Be sure to include the Passport Application/Locator number on the form
    • Special Instructions for minors
      • Complete the form with the minor's details (i.e., Name, DOB, SSN)
      • Parents/Guardians may sign for minors and must include their name and relationship to the applicant in the comment section.
  • Proof of your upcoming travel
    • I.e., airline or cruise itinerary with applicant name and departure date listed
    • If traveling overland to Canada - accommodation booking, a ticket to an event, or an invitation
  • The reason for your travel
    • Leisure, study, business, or life-or-death emergency
      • Proof of your emergency if applicable (i.e., a letter from medical professionals, death certificate; please provide an English translation)
  • If you have not yet paid both the expedited service and return shipping fees
    • Include an attached document with credit card payment information (CC#, name on card, expiration, CVV, and billing zip code). Please note on your release form that you are including this information to pay the expedited fees.
    • Or contact the National Passport Information Center to make payment for expedited service and shipping over the phone (1 (877) 487-2778). Please note on your release form that you paid the fees over the phone.
  • If the Passport Agency previously requested additional documents or a replacement photo
    • Provide the date you submitted your response and the tracking number for your mailing (if possible)

One of Congressman Sarbanes' most important responsibilities is helping with federal agencies. Though the Congressman cannot override the decisions made by a federal agency, he and his staff can work with you to find solutions or cut through the red tape.

Regardless of which agency you need help with, Congressman Sarbanes' office requires a completed privacy authorization form to comply with the provisions of the Privacy Act of 1974. Completing and signing the Privacy Act Release Form here will provide the office with the information to reach out and help complete your request.
